When you've made the decision to participate in ProPath

Members and Associates

1. Complete this registration form by December 30, 2009 to hold your place
2. Upon notification of acceptance, you'll have two payment options
   • Complete payment by December 30, 2009 qualifies for the discounted price of $950
   • Installment payments of $400 each (total cost $1,200)
     • First  $400 installment by December 30, 2009
     • Second $400 installment on January 29, 2010
     • Final $400 installment on March 30, 2010
